LPN/RN

Full-time position to coordinate and provide care to behavioral health patients on an Assertive Community Treatment team. This person may call-in prescription orders, monitor medication and supplies, distribute medications, ensure proper linkages to resources, schedule, room patients and perform patient checks, and patient education. The person in this position is responsible for following all infection control procedures. The person works within a team supporting the patient and other staff involved in the patients treatment.
